when i click on import now button . it is giving like this error.

what module are you importing into? The first item sounds like the last name is a required field in the module you are trying to import to, and that this field is not mapped or not included on your csv.
For the datetime and date fields, looks like these dates are not properly formatted on your import document. Usually, the format on the import document should match the format for the date and times specified under My Account area for the User who is trying to import. You can override this by clicking the 'Show Advanced Options' button in the import mapping screen to set a different date and time format.
